Pilot Scheme on Home Care and Support for Elderly Persons with Mild Impairment (SEMI)

Introduction

 “Pilot Scheme on Home Care and Support for Elderly Persons with Mild Impairment” (“Pilot Scheme”) is funded by Community Care Fund and administered by Social Welfare Department to provide home care and support services for low-income elderly persons with mild impairment. 

Service Target

  • Hong Kong residents aged 60 or above,
  • Those living in Shamshuipo and Kwun Tong districts,
  • Being assessed as mild impairment under a designated assessment tool,
  • Those waitlisting for Shamshuipo Integrated Home Care Service Team (SSPIHCST) or Wan Hon Integrated Home Care Service Team (WHIHCST) (ordinary cases),
  • Monthly household income not exceeding a specified percentage of the relevant Median Monthly Domestic Household Income .

Service Boundary

Shamshuipo and Kwun Tong Districts

Application

  • SWD will invite those waitlisting for Integrated Home Care Service to apply for the Pilot Scheme.
  • SWD will assess the application and inform the elderly the result.

Withdrawal

Service users are required to notify the Case Manager by phone or in-person at least one month before withdrawal.

Service Contents

Provision of meals and home care services including personal care, simple nursing care, physical exercise, general household duties, escort, environment risk assessment and health management, purchase and delivery of daily necessities, carer support services, etc.

Service Charge

Service charge is based on a sliding scale with reference to financial situations of applicants.  It may be revised annually.
